I have found a "Hallowed Bandari's Helm of Immortality" which as far as I know should be sufficient for the Cystalline mask formula. But it is not. Anyone knows about this something? Do I need a specific rare prefix?

On a side note, it happened in a mod called Soulvizier, which is based on your Underlord. So far every mythic formula worked that is why I thought this helm might have been disabled in the original UL or something like this. But it still can be a bug in Soulvizier.
I tested all the formulas before release, so I have no other explanation. You could try just loading up Underlordemus to complete your item there; I think the formulas are a part of that record set.
Okay, I am probably gonna test the formula with your original Underlord and if that solves the problem then I am gonna report this bug to amgoz1 (the creator of soulvizier mod).
Also, I was never able to find the cystalline mask formula (I got this one from the vault). Whenever I found a "Mythic Formula - Cystalline Mask" it was actually the Galefury (picture below) and not the good old Cystalline mask. This raised the question for me if the old Cystalline mask have been disabled or I was just simply unlucky with finding one.
