Heya.

The wife and I both played through the demo several times and are really looking forward to the game's release now. I haven't played the games Titan Quest is based on, so the genre is mostly new to me.

My main interest will be running through the campaign with my wife, and I've also piqued the interest of two of my coworkers.

I'm curious though, how is the splitting of loot handled in multiplayer?

Say the four of us down a group of mobs, and a very nice helm drops three of us could use. Is there an automated system to roll on the item (such as World of Warcraft has executed so cleanly)? Is there at least the command to "/random 100" in order to choose a winner? Or is it first click, first serve? Among friends, it would work, but could there be a better method?

AFAIK, there is no item drop reservation, so you and your buddy players have to stick with the method: be nice and cooperative and give the item to the character in your group that can make most use of it.
That's how I use to play MP. Even if you don’t know yet the people you are playing with or your buddies are selfish at first, you will be surprised how kindness can be infectious.

P.S.: If you still can't decide, give the item to character of the player who can't handle the game so well like the other players.

To answer your question, in Diablo 2 in which footsteps Titan Quest is aiming to follow, when loot dropped from a monster it was first clicked first served. This could cause for some frustrations when you alone had been fighting a monster down from 100% health to only 10% health, and then a 2nd person (usually a melee type who stood close to the monster and therefore also to the drop) would make the killing blow, and be the first to grab the drop.

I have played D2 with friends, and also with my wife, for about 2 years straight, and sometimes I felt the urge to go solo a bit, even in a MP coop game, just so I could also get some loot from monsters I killed myself. This because I usually played a ranged character (spellcaster or amazon type) while my friend, and also my wife, prefer to play the melee type to just hack and slash away from up close.

This was one thing I liked so much in GuildWars, where every item that dropped was randomly assigned to a player, and for a minute or so only that player could pick it up. If he/she didn't, then the assignment expired and anyone could pick it up.

But in Titan Quest it will be the D2 style, where any drop is free for all and who clicks on it first will get it.
