I found 2 till now. Here they are:

I found it really funny. :) Got it in a cave near the City of Lost Souls (or what name it has) from a hero monster.

I found it on a crab hero on the beach near the starting town of act IV.

Post your own and if you can specify the type of hero monster that dropped it along with the location. Me thank you for your niceness.
