It's not.
They just get some vitality resistance and a (bosses-only) 96% resistance to %health reduction , you need to nuke both resistances for it to work. And sapros does exactly that , so the boss should start "bleeding" quite noticeably from plague. But I wouldn't bother with %health reduction against anything but bosses.
