Quest not updating

I tried searching for this but found no fix or answer that was definitive.

On my character the first two parts of Act IV are completely blank, even though I went through them. I've gotten to the part to give Medea the eye of the gray sisters but I am unable to talk to her, she has the exclamation mark over her head but she doesn't highlight and I am unable to click on her. My girlfriend who has played along with me, her quests have all updated fine except now when she talks to Medea to give her the eye, she gives the speech response as if she's still waiting on her to retrieve it.

Also, two side quests I have will not update, this is happened in the past but me and my girlfriend have been able to bypass it by one of us turning the quest in and it updating for the other. But now we both cannot turn in the quest to give the eye to Medea, and we cannot further the main story.

We're using the fan patch and have it updated to the most recent version, would continously playing through the same game on different versions of the game mess it up?

Or does anyone else know some way to fix this...
