Explosive Arrow Failed?

Has anyone noticed that the Explosive Arrow will sometimes fail?

A few times when it seemed to make positive contact with a monster it did not explode. This is not when it missed or struck an object.

I played it quite a while and i don't think i remember seeing it. I will play some more with checking more closely to that issue!

I haven't noticed anything either.
